    Hi Dr V, I am at a crossroad of considering to go back on Plaquenil . A year ago I proposed to go off Plaquenil after being on it for 22 years.

    Hydroxychloroquine Plaquenil is considered a disease-modifying anti-rheumatic drug DMARD. It can decrease the pain and swelling of arthritis. It may prevent joint damage and reduce the risk of long-term disability. Hydroxychloroquine is in a class of medications that was first used to prevent and treat malaria. Following a single 200 mg oral dose of PLAQUENIL to healthy male volunteers, the mean peak blood concentration of hydroxychloroquine was 129.6 ng/mL, reached in 3.26 hours with a half-life of 537 hours 22.4 days. In the same study, the plasma peak concentration was 50.3 ng/mL reached in 3.74 hours with a half-life of 2963 hours 123.5 days. An analog of chloroquine – hydroxychloroquine – has a long half-life 32–56 days in blood and a large volume of distribution 580–815 L/kg. The therapeutic, toxic and lethal ranges are usually considered to be 0.03 to 15 mg/l, 3.0 to 26 mg/l and 20 to 104 mg/l, respectively.

    If I had joint or muscle aches, I took care of it by resting, Aleve and ice. At my recent eye exam the doctor mentioned I had reached the plaquenil toxicity level and that my retina cells could die (which do not regenerate! My proposal was based on the fact that I had not had any major inflamation that completely shut me down or keep me from being active.

Urine hydroxychloroquine levels were still detectable after 3 months with approximately 10% of the dose excreted as the parent drug. The 1-2 months is correct. It’s about 45 days before it starts to go out of your system. Remember it is cumulative, and took time to build up for an effect when you started taking it. It has an enormous half life. That’s why I laugh when some people’s surgeons tell them stop taking it 2 weeks before surgery. It does nothing. The normal dose of Plaquenil is 1 tablet a day 200mg/day. Some doctors recommend 2 a day 400mg/day but we find that 1 a day suits most people – the dose can always be increased for a flare. One of the attractions of this medication is its long ‘half-life’ – it stays in the system for days & weeks.
